Payment is required BEFORE your appointment, either by credit card (via online booking system), or internet banking. Any appointment cancellations must be made at least 24 hours in advance. Any cancellations made within 24 hours will incur a fee of 50% of the appointment cost. If you do not attend an appointment a 50% ‘did not attend’ fee will be charged.

I consent to treatment, and understand that a full verbal explanation of my treatment plan will be given at time of consultation, and a summary will be emailed to me within 24 hours of my appointment.

I understand that I am entitled to have a support person(s) with me when undergoing assessment or treatment, except where safety may be compromised or other patients rights may be unreasonably infringed on. I also understand I am entitled to advocacy services, and can access these through Health and Disability Service (Ph 0800 11 22 33).
Consent for payment
I understand that:
